Following an informal chat with a member of our team, a home visit will be arranged with/without family members, informal carers and any other healthcare professionals as appropriate.

A suitably qualified member of the team will visit you in your home to explain how our service works and get to know you.  During this visit, Ashbee Home Care will form an assessment of your individual needs and wishes, welcoming family and informal carers to contribute to your  tailored care plan if wished.

Step 4: Daily care will commence

A Care Plan will be created based on your individual needs and requests, as discussed during your home visit.  This care plan will be reviewed by yourself and any other contributors to your care, in order to ensure you feel content with the planned care package.  


Once a care plan has been finalised, a start date will be agreed.

Your personalised care will begin, whereby care staff will visit you each day, carrying out tasks agreed within your care plan.  You are welcome to contact Head Office at any point, should your needs change or you have any concerns / wishes to be discussed.

Your Care Plan will be reviewed annually, or sooner should any concerns or wishes be raised by yourself, family members or your care team, or if there is a change in your health condition, naturally requiring adjustments in care.
